Description

The Community Service Award application requires the completion of an online survey, the link to which will be available on the event registration confirmation page. It includes a list of details needed for us to order the plaque. This must be completed by the end of each quarter for submission in that quarter (March 31, June 30, September 30, Dec 31)

Please use the name of the person ordering the awards on this page, as this is just for billing purposes. The award recipient info should be filled out on the form that is linked on the confirmation page. Please fill out the form for each recipient your are ordering an award for.
